Social Services provides supportive counseling, information and referrals regarding community resources and assistance planning your post-hospital care.

Going Home
After your physician writes a discharge order stating that you are ready to go home, the nursing staff will assist you in making arrangements to leave the hospital. If you need the services of the discharge planner, please call the Social Services Department.

Thomas Hospital offers a personal response system for those who need an easy, reliable way to contact help in an emergency once you've gone home. There is a minimal monthly fee. Call extension 990-1518 for information.
